Imagine having a team of knowledgeable and caring healthcare professionals collaborating with you to address health needs, overcome challenges and achieve your dreams. Healthcare as a Team Sport does just that for athletes, who require optimal health to succeed in sport. The keys to this model of care are having a team of healthcare professionals collaborating with athletes, trusting relationships between all teammates and focusing on the 5 Areas of Proactive Health.
Athletes have this care and Partnership for Proactive Health is on a mission to bring Healthcare as a Team Sport to everyone.

Our Programs. Introduce Healthcare as a Team Sport in the community.
Working towards our goal of bringing Healthcare as a Team Sport from athletes to everyone, starts with addressing significant health issues facing our community through individual programs centering on the 5 Areas of Proactive Health. Given the staggering statistics around health – worst outcomes, highest cost, low trust – the need is great and we’re here to serve.
