What. 4th Trimester Care brings Healthcare as a Team Sport to families optimizing maternal health.

4th Trimester Care begins prenatally proactively building trusting relationships and planning for the care + support a family’s wants and needs after baby arrives. Continuing into the 4th trimester bringing excellence in care through a combination of in-home and virtual care + support. Surrounding the family with the support they desire, centering on the 5 Areas of Proactive Health and ensuring every family can thrive.

How. Establishing excellence in maternal healthcare starts with empowering teams, building trust and inspiring engagement.

Team

Just like in sports, the first step is building our team. Our focus is collaborations with existing entities, including maternal health physicians, health systems, companies and third party payers. It all begins with a conversation.

Trust

Building relationships with collaborators and their community to understand the needs. Using our care model and each community’s needs as a guide creating a pathway to integrate 4th Trimester Care.

Engage

Collaboratively integrating 4th Trimester Care into existing entities for delivery in local communities. Demonstrating improved outcomes will be key. Together we will write a new chapter on maternal health, helping all families thrive.
